REGISTERED NURSE

QUALIFICATIONS

  • Must be a graduate from an accredited School of Nursing.
  • Must be licensed in the state, as a Registered Nurse.
  • At least one (1) year experience in a community/home health agency or in a hospital setting, is preferred.
  • Must have knowledge of Medicare and Medicaid guidelines.
  • Must have a working knowledge of home healthcare, and the principles and techniques of professional nursing, and required documentation that pertains to it.
  • Must have a current CPR certification. Online certification is not accepted.
  • The ability to communicate, both, verbally, and in writing, is required as frequent communication, by telephone, and in writing in English, is required.
  • Must have a criminal background check.
  • Must have a driver’s license and be willing, and able, to drive to patients’ residences.

DUTIES & RESPONSIBILITIES:

  • To provide nursing care, in accordance with the patient’s plan of care, to include comprehensive health and psychosocial evaluation, monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities, and direct nursing care.
  • Coordinate total patient care by conducting comprehensive health and psychosocial evaluation, monitoring the patient’s condition, promoting sound preventive practices, coordinating services, and teaching and training activities.
  • Evaluate the effectiveness of nursing service, to the patient and family, on an ongoing basis.
  • Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.
  • Prepare, and present, patient’s record to the Clinical Record Review Committee, as indicated.
  • Consult with the attending physician, concerning alterations of Patient Care Plans, checks with the appropriate supervisor, and makes changes, as appropriate.
  • Coordinate patient services.
  • Submit clinical notes, within seventy-two (72) hours, and progress notes and other clinical record forms outlining the services rendered.
  • Submit a tally of patient care visits made each day.
  • Participate in case conferences, discuss with the supervisor problems concerning the patients, and how they may best be handled.
  • Discuss, with the appropriate supervisor, the need for the involvement of other members of the health team, such as the Home Health Aide, the Physical Therapist, the Speech Therapist, the Occupational Therapist, The Medical Social Worker, etc.
  • Participate in the patient’s discharge planning process.
  • Adhere to federal, state, and accreditation requirements, including Medicare and Medicaid regulations.
  • May be requested, by Clinical Manager, to fill in for the other nurses.

LICENSE PRACTICAL NURSE

QUALIFICATIONS:

  • Graduation from an accredited School of Practical Nursing.
  • Must be licensed in the state as a Licensed Practical Nurse.
  • At least one (1) year experience in the Home Health Agency or in a hospital setting. Home health experience preferred.

DUTIES & RESPONSIBILITIES:

  • Provide skilled nursing care, under the direct supervision of the Registered Nurse.
  • Perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care.
  • Is responsible, and accountable, for making decisions that are based on the individual’s nursing experience and educational preparation.
  • Assist the Registered Nurse in carrying out the plan of care.
  • Assist the Registered Nurse in performing specialized procedures, as directed, by the Registered Nurse under physician’s orders.
  • Assist with the preparation, implementation, and continuing evaluation of the patient care plan.
  • Recognize, and understand, the effects of social and economic problems upon patients and provide for the emotional and physical comfort and safety of patients.
  • Help teach the patient the appropriate self-care techniques.
  • Foster cooperative effort, among personnel, by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences.
  • Assume responsibility for personal and vocational growth and development, through active participation in nursing organizations and participating in in-service programs and other on the job learning.
  • Prepare clinical and progress notes (not to include admission).
  • Observe, record, and report, to the appropriate person, patient symptoms, reactions, and changes including:
    • The general physical and mental condition of patients, signs, and symptoms, which may be indicative of changes in condition.
    • Stresses in human relationships, between the patients and staff personnel, and between patients and their families and visitors.
  • ​Assist with the rehabilitation of patients, according to the medical care plan, by encouraging the interests and special aptitudes of the patient and encouraging patients to help themselves using their own capabilities.
  • Further assist, in rehabilitation, by reporting any abnormality in the patient’s range of motion, body mechanics, or body alignment.

HOME HEALTH AIDE (CNA)

QUALIFICATIONS:

  • Preferably a high school diploma or equivalent.
  • Successful completion of one of the following:
  • Must comprehend the basics of personal care, housekeeping, and meal preparation.
    • A minimum 75 hours training and competency evaluation program; or
    • A competency evaluation program that meets the requirements; or
    • A nurse aide training and competency evaluation program approved by the state, and is currently listed in good standing on the state nurse aide registry; or
    • The requirements of a state licensure program that meets the provisions
  • Must understand and respect client’s, including ethics and confidentiality of care.

  • Must have a criminal background check.
  • Must be able to communicate clearly, both verbally and in writing, in English.

DUTIES & RESPONSIBILITIES:

  • Home Health Aide services may also be provided by a CNA, subject to the same supervision described below. If necessary, they may also be provided by an LPN or RN.
  • Provides personal care and related services in the home, under the direction, instruction and supervision of the staff nurse and the Nurse Supervisor.
  • Tasks to be performed by an HHA must be assigned by, and performed under, the supervision of an RN, who will be responsible for the patient care provided by the HHA.
  • Under no circumstances may an HHA be assigned to receive, or reduce, any intravenous procedures, or any other sterile or invasive procedures, other than rectal temperatures.
  • Follows the plan of care to provide, safe, competent care to the client.
  • Helps the patient maintain good personal hygiene and assists in maintaining a healthful, safe environment.
  • Plans and prepares nutritious meals, markets, when instructed to do so by the nurse.
  • Assists the patient with ambulation, as ordered by the physician, and approved and supervised by the nurse.
  • Assists the therapy personnel, as needed, with rehabilitative processes.
  • Encourages the patient to become as independent as possible, according to the nursing care plan.
  • Attempts to promote a patient’s mental alertness, through involvement in activities of interest.
  • Reports any change in the patient’s mental, or physical, condition, or in the home situation, to the staff nurse, or to the Aide supervisor.
  • Performs routine housekeeping tasks, as related to a safe and comfortable environment for the patient, as instructed by the professional nurse.
  • Prepares a visit report, promptly, and incorporates the same in the clinical record, weekly.
  • Confirms, on a weekly basis, the scheduling of visits so that other necessary visits, by staff members, can be coordinated.
  • Attends in-service, as required by regulation.
